目的 观察个体化限制热量高蛋白饮食对2型糖尿病患者人体成分及糖脂代谢的影响,为进一步探讨保护瘦体重提供依据。方法 选取内分泌科住院治疗的2型糖尿病患者137例,其中常规糖尿病饮食者68例(对照组),个体化限制热量饮食方案者69例(观察组),每月门诊或远程随访,观察两组患者干预0月、3个月、6个月后的人体成分及糖脂代谢变化。结果 干预3个月及6个月,对照组与观察组空腹血糖和糖化血红蛋白均下降,且观察组低于对照组,差异均有统计学意义(P<0.05);干预3个月后,对照组与观察组胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白均下降,但与干预0月时差异无统计学意义(P>0.05);干预6个月后,两组HbA1c、空腹血糖、胆固醇水平均下降,观察组胆固醇、甘油三酯水平低于对照组,差异均有统计学意义(P<0.05)。干预3个月及6个月,观察组体重指数、体质量、骨骼肌、脂肪含量下降,与对照组差异均有统计学意义(P<0.05)。结论 个体化限制热量高蛋白饮食干预有利于糖尿病患者控制体重,同时可改善糖尿病患者糖脂代谢障碍的程度,从而改善血糖水平。
Abstract:Objective To investigate the effects of individualized, high-protein, calorie-restricted diets on body composition and glucose-lipid metabolism in patients with type 2 diabetes, providing a basis for further exploring the preservation of lean body mass. Methods A total of 137 inpatients with type 2 diabetes from the endocrine department were selected, including 68 on a conventional diabetes diet(control group) and 69 on an individualized, calorie-restricted diet(observation group). Monthly outpatient or remote follow-up was conducted to observe changes in body composition and glucose-lipid metabolism before intervention and at 0, 3, and 6 months after intervention. Results After 3 and 6 months of intervention, both fasting blood glucose and glycated hemoglobin in the control and observation groups decreased, with the observation group being lower than the control group, and the differences were statistically significant(P<0.05). After 3 months of intervention, cholesterol, triglycerides, high-density lipoprotein, and low-density lipoprotein decreased in both groups, but the differences were not statistically significant compared to before intervention(P>0.05). After 6 months of intervention, HbA1c, fasting blood glucose, and cholesterol levels decreased in both groups, and the observation group had lower levels of cholesterol and triglycerides than the control group, with the differences being statistically significant(P<0.05). After 3 and 6 months of intervention, the observation group showed a significant decrease in body mass index, body weight, skeletal muscle, and fat content, with statistically significant differences compared to the control group(P<0.05).Conclusions Individualized, high-protein, calorie-restricted diets are beneficial for patients with diabetes to control their body weight, improve glucose-lipid metabolism, and thereby improve blood glucose levels.
